New Patient Registration
We would be pleased to register anyone who lives within the practice area. The red line on the map indicates the practice boundary.
Patients can register with either the Princes Street or the Halkirk Surgery but usually register with the surgery nearest to them for convenience.
You can register:
- By downloading the form HERE for completion. Please print a copy, sign the form and return to reception in person.
- In person at Reception at the surgery of your choice
- By telephoning the surgery and asking our Receptionists to send registration forms to your home address
You may be asked for proof of identity (passport or driving licence with photo ID) and proof of address (any utility bill) so please ensure that you take this with you. If you have a Medical Card, please bring this with you too.
You will need to complete one registration form for each member of your household. Once registered, we require you to have a new patient check, consisting of height/weight measurements, blood pressure check and details of both patient and family history.
This initial check-up is usually performed by the Practice Nurse, and this, combined with the completed new patient questionnaire (please print and complete BOTH documents below), helps the doctor to assess your general health and to familiarise themselves with your medical history, while waiting for your full medical records to arrive.
Change of Details
If you change your name, address (including postcode) or telephone number please notify the surgery as soon as possible (see Contact Details form at bottom of Home page)
